About ConsentForms
ConsentForms is software for collecting signed consent, waivers, and releases — a link or QR, an e-signature, and an encrypted copy you can still find in October.
The product is built by Clint Reed. He also runs shop-facing signing sites, including pmusign.com, lashsign.com, and inkdsign.com. Those businesses still drown in paper when the desk is busy. ConsentForms is the version that is not tied to one trade: any studio, camp, gym, or photographer can publish a form, share a link, and keep a hashed record.
What we are
A web app: drag-and-drop builder, templates, Jarvis (an AI that drafts a form from a description or a photo of your paper sheet), QR codes, iPad kiosk, Stripe or Square on the same form, and signed PDFs. Submissions are envelope-encrypted. The database stores pointers and hashes, not the answers in plain text.
What we are not
We are not a law firm. Templates are starting points. Have an attorney review waivers for your state. We are not a HIPAA product and we do not sell this as a medical-records system. We are not DocuSign for enterprise contract workflows.
