Consent forms for salons and suites
Brows, lashes, color, and facials — the aftercare is on the form they already signed.
Suites and salons collect health questions and aftercare on paper that never makes it back to the booth. A link in the booking text fixes that. Permanent makeup, microblading, and lash extensions each have their own template.
Use kiosk mode on a shared iPad at the front, or a QR on the mirror. Pay on the same form if you take a deposit through Stripe or Square.
