Free permanent makeup consent form

Clients read the risks and aftercare on their phone before you open the pigment — not on a clipboard after numbing cream is already on.

Permanent makeup is a cosmetic tattoo. Insurers and state boards expect a signed record: who they are, what you are placing, that they disclosed health issues that affect healing, and that they will follow aftercare. A verbal 'you're good' is not that record.

This template is the public version of the ConsentForms PMU consent. Edit the aftercare to match brows, liner, or lips. Put your legal studio name in place of [Organization Name]. It is a starting point, not legal advice, and it is not a medical/HIPAA product.

Send the link with the deposit text, or print a QR next to the bed. Kiosk mode on a shared iPad starts a fresh form after each signature.

What this form includes

  • Client identity and contact
  • Treatment area and description
  • Health questions that affect healing
  • Aftercare you can edit
  • Release language and e-signature

How to use it

  1. Open the permanent makeup template and swap in your studio name and aftercare.
  2. Publish. Text the link with the appointment reminder or tape the QR at the bed.
  3. Download the signed PDF if a client or your insurer asks. The payload is envelope-encrypted.

Questions

Is a PMU consent form different from a tattoo consent?

The structure is similar because PMU is a cosmetic tattoo. This template is worded for brows, liner, and lips, including aftercare that matches those placements.

Can I use this for microblading too?

Yes. There is also a dedicated microblading template in the gallery. Either works if you edit the treatment description and aftercare.

Will this work on a phone with numbing cream on?

Have them sign before you start. Send the link when they book so the form is done in the waiting area.
