Free liability waiver template
People sign the waiver on the way in. You keep a hashed record instead of a drawer of paper.
A liability waiver records that someone understood an activity involves risk, chose to take part, and released ordinary claims. It does not hide gross negligence, and some states limit what you can waive.
Start here if you do not already have a sport-specific form. Gyms, climbing, youth sports, rentals, and events each have a closer template in the gallery. Name the activity and the risks you actually run — generic 'activities' language is weaker than 'indoor climbing, including falls and equipment failure.'
Share a link, print a QR, or open kiosk mode on the front-desk iPad. This is a starting point, not legal advice. Have an attorney review the waiver for every state you operate in.

Questions
Are electronic waivers valid?
They are widely used when you can show the person saw the text, signed it, and you kept an unaltered copy. ConsentForms encrypts the payload and stores a content hash of the form version.
Can I use one waiver for every location?
You can share one form, but the legal name and activity should match the location. Workspaces (on paid plans) let you separate brands or gyms.
What about kids?
Many youth programs need a parent signature. Several activity templates ask if the participant is a minor and collect the child's name. Enforceability varies by state.
How do I get rid of the clipboard at the door?
Print the QR from Settings. New members scan, sign on their phone, and you already have the PDF by the time they reach the desk.
