Free gym and fitness waiver
New members sign on the way in. You keep a hashed record instead of a binder behind the desk.
A gym waiver records that someone understood weights, classes, and equipment involve risk, and that they release ordinary claims. It will not hide gross negligence, and some states limit what you can waive.
Name the real activities: free weights, turf, classes, sauna. Generic 'fitness activities' is weaker. This template is a starting point, not legal advice — have an attorney review it for every state you operate in.
Bookmark kiosk mode on the front-desk iPad so each new member starts a blank form. Existing members can get a link in the welcome email.

Questions
Are electronic gym waivers enforceable?
They are widely used when you can show the member saw the text, signed it, and you kept an unaltered copy. ConsentForms stores a content hash of the version they signed.
What about class packs or day passes?
Use the same waiver for members and drop-ins. Add a dropdown for membership type if you want that on the record.
