Free child travel consent form

When a child flies with one parent, a grandparent, or a school group, a signed travel consent letter is what officers and airlines actually ask to see.

There is no single official worldwide blank. Canada, U.S. Customs, and airlines publish examples: child's identity, accompanying adult, destinations and dates, and the consenting parent or guardian. This template puts those fields in an online form you can send to the other parent and print for the airport.

Use it for a child traveling with one custodial parent, with another adult, or with a group. For international trips, add a notary block in the builder if the destination or airline wants one. Original printed copies are taken more seriously than a phone screenshot at a border.

This is not legal advice and not a government form. Check the airline and the destination before you travel. If parents are separated, both may need to sign.

What this form includes

  • Parent or guardian details
  • Child's passport or ID number
  • Accompanying adult (optional if traveling alone)
  • Destinations and travel dates
  • Emergency phone
  • Consent statement and signature

How to use it

  1. Start from the minor travel template. Use the child's legal name as it appears on the passport.
  2. Share the link with the non-traveling parent. Add a second signature field if both must sign.
  3. Download the signed PDF, print an original, and pack it with the passport.

Questions

When do I need a child travel consent form?

When a minor travels without both custodial parents — with one parent, a relative, a coach, or a school group. Border agencies and some airlines will ask for written permission.

Does it have to be notarized?

Often recommended for international travel, sometimes required. Domestic trips may accept a clear signed letter. Add a notary block if you need one, then print the original.

Can I fill this on a phone?

Yes. Send the other parent a link. They sign on any device. You download a PDF to print for the airport.

Is this the official government form?

No. Governments generally do not require a specific blank; they want the facts. This template collects those facts. Check the destination's current guidance.
