Free tattoo consent form template
Clients sign the tattoo consent and aftercare on their phone in the waiting area — not on a clipboard after you've already started the stencil.
Walk-in studios lose time and signatures when the consent sheet is in a drawer. A tattoo consent form should be ready before the needle: who they are, what they're getting and where, that they understand healing risks, that they are not impaired, and that they will follow aftercare.
This page is the public version of the ConsentForms tattoo template. It is written for searchers who need a tattoo waiver today, not a generic form builder homepage. Use the live template, or describe your shop's aftercare to Jarvis and he will draft a custom form (two free demos before you create an account).
Replace [Organization Name] with the legal name on your shop lease. Add initials on extra paragraphs if your insurer wants them. This is not legal advice and does not replace a lawyer's review of your state's tattooing rules, including age limits.
What this form includes
- Client name, contact, and date of birth
- 18+ (or parent consent) confirmation
- Tattoo description and placement
- Not-impaired confirmation
- Aftercare instructions you can edit
- Release language and e-signature
How to use it
- Open the tattoo consent template and swap in your studio name and aftercare.
- Publish, then print the QR from Settings and tape it at the desk — or send the link with the booking text.
- Each signature stores a hashed form version, time, and IP. Download the signed PDF whenever a client (or an insurer) asks.

Questions
Is this tattoo consent form free?
Yes. Copy the template into a free ConsentForms account, or ask Jarvis to build a custom version (two demos before signup). Free includes ConsentForms branding, 1 seat, 1 workspace, and 100 signatures a month.
Can I collect tattoo consent on an iPad at the counter?
Yes. Settings has a kiosk URL for a shared iPad and a QR for the client's own phone. Kiosk starts a fresh form after each signature.
What if a client is under 18?
Many places ban tattooing minors. Where it is legal, a parent or guardian must usually sign. Add a guardian block in the builder, or refuse the appointment.
Do you store the signed tattoo waiver?
Yes. The payload is envelope-encrypted. The database keeps pointers and hashes so you can prove what was signed later. You can download a PDF copy anytime.
