Free photo release form template

Get permission to photograph and publish before you post the recap — on a phone, not a clipboard in the parking lot.

A photo release is the difference between a gallery you can use and a folder you are afraid to post. It should say who is in the frame, who may use the images, and whether that includes ads and social media or internal use only.

This template is the adult photo and video release. For kids' sports, camps, and schools, switch to the photo release for a minor so a parent signs. For a paid commercial shoot, start from the model release instead.

Put your studio or organization legal name in the document. If you promise not to use last names or paid ads, say so in the builder. This is not a copyright assignment and not legal advice.

What this form includes

  • Signer name and contact

How to use it

  1. Open the photo release template and name your studio.

Questions

Do I need a photo release for every person in a group shot?

For marketing use, yes if they are recognizable. Events often collect a release at registration so you are not chasing people after the fact.

Photo release vs model release?

Use a photo release for everyday events, camps, and studios. Use a model release when someone is posing for commercial or editorial work, often with compensation.

Can parents sign for children?

Yes — use the photo release for a minor. The child's name and the guardian's authority sit on the same signed record.

Will this work on a phone at a soccer game?

Yes. Send the team a link or show a QR on a coach's phone. They sign in place. You keep the encrypted record.
