ConsentForms vs Google Forms

A Google Form with a 'type your name' box is not a waiver. You need a signature image, a frozen copy of the text they agreed to, and a way to prove it later.

Google Forms is excellent for RSVPs and quizzes. It is the wrong place to store a liability waiver. There is no drawn signature, no published-version hash, and no kiosk reset for the next person in line.

ConsentForms publishes a version, collects a signature on any phone, and stores an encrypted payload plus a hash of that version. If you edit the waiver tomorrow, last week's signatures still point at last week's text.

If you only need a headcount, stay on Google. If an insurer or parent will ask 'what did they sign?', use this.

ConsentForms

  • Drawn or typed e-signatures
  • Tamper-evident hash of the form version
  • Signed PDF you can download
  • QR and kiosk for the door

The other option

  • Free and familiar
  • Fine for RSVPs, not for waivers

Questions

Can I import a Google Form?

Paste the public link into Jarvis. Review every question — Google Forms does not have a real signature field, so Jarvis will add one.
