Free event participation waiver

The waiver is in the ticket email. By the time they reach the door, you already have the signature.

Races, festivals, and ticketed events need a signature that matches the person at the gate. Collecting it at the table creates a line. Collecting it in the confirmation email does not.

Name the event, the date, and the actual risks (crowds, weather, physical activity). This template is a starting point, not legal advice.

Put the QR on the poster at will-call for people who did not finish the email form.

What this form includes

  • Event name and assumption of risk
  • Release of ordinary claims
  • Signer identity
  • E-signature

How to use it

  1. Start from the event waiver. Name the event and the risks that are real for that day.
  2. Paste the link in the ticket or registration email.
  3. Use kiosk at the gate for walk-ups.

Questions

Can each ticket holder sign separately?

Yes. Send the same link to every registrant. Each submission is its own signed record.

What about kids at a family event?

Use parental consent or a youth-sports waiver so a parent signs. Do not let a child sign an adult waiver.
