How to collect waivers with a QR code
The clipboard at the door is a line. A QR on the counter is the same waiver without the stack of paper.
Client's own phone
Publish the form. Open Settings and print or download the QR. Tape it at eye level. They scan, sign, and you already have the submission when they walk up. This is the default for tattoo shops, gyms, and salons.
Shared iPad (kiosk)
If they should not use their own phone, bookmark the kiosk URL on a counter iPad. Kiosk starts a fresh form after each signature so the next person does not see the last person's name.
Send the link before they arrive
Camps, photographers, and ticketed events should put the link in the confirmation email. The QR is then only for walk-ups.
Questions
What if the Wi-Fi is bad?
The form loads from the internet. Use the shop Wi-Fi or their cell network. Kiosk on a wired iPad is the fallback when phones will not connect.
Guides and templates are a starting point, not legal advice. ConsentForms is not a HIPAA product.
