Free liability waiver template

Release of liability and assumption of risk for activities.

A liability waiver is the form people sign before they climb, train, take a class, or walk into a venue. It records that they understood the activity involves risk, chose to take part, and released the organizer from ordinary claims — not from gross negligence.

This general waiver is the right start when your activity does not already have a sport-specific template. Replace the activity description and the risks. Gyms, events, rentals, and youth sports each have a dedicated variant in the gallery if you want language closer to the activity.

Waivers are state-specific. Some states limit what you can waive. This template is a starting point, not legal advice. Put your legal name in place of [Organization Name] and have an attorney review it before you rely on it.

Questions

What should a liability waiver include?

A description of the activity, the inherent risks, an assumption of those risks, a release of claims (except gross negligence or willful misconduct), rules the participant agrees to follow, identity fields, and a signature with the date.

Does an electronic waiver hold up?

Electronic signatures are widely used for waivers when you can show the person saw the text, signed it, and you kept an unaltered copy. ConsentForms stores the payload encrypted and records a content hash of the form version.

Can a parent sign a waiver for a child?

Often yes for youth activities, and many venues require it. Use a template that asks whether the participant is a minor and collects the child's name. Enforceability varies by state.

How do gyms collect waivers at the door?

Print the QR from form Settings or bookmark the kiosk URL on an iPad. New members sign on their phone in line. You are not handing out clipboards.
