Free photo release form template
Permission to photograph, record and publish someone's likeness.
A photo release is permission to take and use someone's image. Without it, a camp, school, or photographer is guessing whether they can post a recap, print a brochure, or run an ad. The form should say who is being photographed, who may use the images, and for what.
This template grants permission to photograph and record, then asks where the images may be used — all media including ads, internal use only, or not at all. Swap in your studio or organization name. For children, use the photo release for a minor instead so a parent signs.
Releases are not one-size copyright transfers. If you need a commercial model release with consideration, start from the model release template. Have counsel review anything you use in advertising.

Questions
What is a photo release form?
A signed document that lets you photograph or record someone and use those images for the purposes listed — often marketing, social media, or internal records.
Is a photo release the same as a model release?
A photo release is the everyday permission form for events, camps, and studios. A model release is usually for a commercial shoot, often with compensation. Use the matching template.
Who signs a photo release for a child?
A parent or legal guardian. Use the photo release for a minor template so the child's name and the adult's authority are on the same signed record.
Can I collect photo releases on a phone?
Yes. Send a link or display a QR. Signers draw or type a signature on any device. You get an encrypted submission and a downloadable PDF.
