Free tattoo consent form template
Client consent and release for a tattoo, with aftercare acknowledgment.
A tattoo consent form is the written record that the client asked for the work, understood the risks, and agreed to aftercare. Studios that skip it are left with a verbal story if a client later disputes swelling, placement, or a design they approved on the day.
This template is built for walk-in and appointment studios. It collects who is being tattooed, what is being tattooed and where, health questions that affect healing, a confirmation they are not impaired, aftercare instructions, and a signature. Put it on a counter QR or send the link with the booking confirmation so the clipboard is gone before they sit down.
Edit the aftercare paragraph, add your studio legal name, and require initials on any extra paragraphs your insurer wants. Templates are a starting point, not legal advice — have your attorney review the waiver language for your state.

Questions
What should a tattoo consent form include?
Client identity and contact, a description of the tattoo and placement, age confirmation, health questions that affect healing, a statement they are not impaired, aftercare instructions, liability language, and a signature with the date.
Can a minor sign a tattoo consent form?
Most jurisdictions require a parent or guardian for anyone under 18, and some ban tattooing minors entirely. Add a parent-signature block if you legally tattoo minors; otherwise keep the 18+ confirmation and refuse the work.
Do I need a paper copy if they signed on a phone?
No. ConsentForms stores an encrypted payload and a tamper-evident hash of the version they signed. You can download a signed PDF anytime. Keep the digital record; you do not need a second paper file unless your insurer asks for one.
Can I put this tattoo form on a counter iPad?
Yes. Settings includes a kiosk URL for a shared iPad and a QR that opens the ordinary client link on their phone. Kiosk resets to a blank form after each signature.
