Sign up, confirm your email, reset a password, and use two-step verification.

An account is what you log into. It holds your forms, team, and billing. Signers who fill out a form do not need an account.

Sign up

  1. Open consentforms.com/signup.
  2. Continue with Google, or type your name, email, and a password.
  3. Check your inbox for a confirmation email and follow the link. Unverified accounts cannot publish forms.

Use consentforms.com/login. Forgotten passwords go through Forgot password — we email a reset link.

Two-step verification

On Account you can turn on two-step. After a password login we ask for a code from your authenticator app. Keep backup codes somewhere safe; support cannot recover an account that lost both the device and the codes.
