Use a template

Browse hundreds of consent, waiver, and release templates and copy one into your account.

The public gallery at /templates is grouped by category — consent, waivers, releases, salon and body art, and more. Using a template copies it into your account so you can edit it. The public original does not change.

From inside the app

1. Open New form and choose Use template, or click a template card on the dashboard.
2. Review the questions. Rename the form, swap your logo on the cover, and add a signature if it is missing.
3. Publish, then share from Settings.
